Form to Participate in Research Explanation of Procedures: You will be asked to respond to a series of questions on the community in which you live, and on your perceived quality of life. Step-by-step instructions will appear on the screen. When completed, you will click the ‘submit’ button at the bottom of the page to save your data. You may withdraw from the survey at any time by closing your web browser's window or by clicking the 'back' button. If you do so, your data will not be saved. Risks and Discomforts: The survey is administered via the Internet. There are no identified risks in participating. Benefits: You may not personally benefit from participation in this research; however your participation could help researchers obtain a clearer picture of how Sustainable Communities impact the mental health of those who live in them. Confidentiality: Your name and other identifying information will not be gathered and your survey will instead be assigned a code number.
